| Child Care- If you are working with a reproductive physician's office outside your county of residence and require child care to attend appointments, you will be eligible to receive reasonable child care assistance/reimbursement. |
| Lost Wages for Surrogate - Due to maternity leave for child birth or bed rest period-computed according to the Surrogate's confirmable income. |
| Lost Wages for Spouse or Significant Other - Payable at the Spouse/Significant Other's confirmable net rate of pay up to a maximum of $1,500.00 total reimbursement of lost wages paid for the duration of the pregnancy for your Spouse/Other to attend *non-routine, medical appointments, procedures and/or birth. |
| Life Insurance -
A policy with the amount of $250,000 coverage will be provided for you. |
Psychological Support-
Psychological support is available to you throughout the pregnancy. We realize not everyone needs monthly one-on-one counseling or twice monthly support group meetings, however we do offer monthly support group meetings which are held by a therapist experienced in infertility and surrogacy issues. We ask that you attend one group meeting every other month and attend a minimum of two individual sessions with our Therapist/Psychologist during the term of the pregnancy or on an “as needed basis”. And, you may attend monthly if you wish, which we feel is really a great way to connect with other surrogates in our program. All therapy/psychological support is at no cost to you. |

a travel allowance and/or daily food expense per diem, any other travel related costs as applicable
(hotel, parking, toll charges, rental car, airfare, train, other public transportation etc.) |
